Trying to modernize wiring in an old house without making it worse

This might be a common situation, but I’m curious how the pros approach it. Older house wiring feels like it always comes with surprises, and I’m trying to understand what “do it right” actually looks like when the current setup is messy. At my relative’s place, they wanted to add a couple new outlets and a dedicated line for a bigger appliance. The electrician opened up a few boxes and immediately started finding odd stuff, mixed wire types, splices that didn’t look great, and a couple circuits that were tied together in a way I didn’t expect. It sounded like the new work was going to be fine, but only after sorting out the old weirdness first. When you walk into an older home job, what are the first compliance red flags you look for so you don’t build new work on top of a bad foundation?
 
Older house work is always a can of worms. First red flags I hear electricians mention are open splices, overloaded boxes, mixed wire gauges on the same breaker, and anything that looks like backstabbed or loose connections. Also any bootleg grounds or weird neutral ground bonding stuff. If the foundation is messy, it makes sense they want to clean that up before adding new runs.
 
Some things I've found very often include:
Two wires under one screw on a device. Sooner or late, one will fall off, hopefully without arcing.

Spring loaded push-in back wired devices going bad. They don't seem terribly reliable.

Splices in the wall with no box- or buried boxes. Plus, soldered splices.

Reversed colors. Peiople who work on cars know Black is "negative" with DC, so they use black as neutral in the house. A house my parents purchased was homeowner wired and 100% backwards on wire colors. I've seen this often.

The scariest one: Aluminum wiring with the wrong type of device attached directly to the aluminum wire. They eventually burn at the connection.

Note that I only work residential freebies, so my house experience is far more limited than the residential electricians here.

yeah, a big thing people don't realize is the Touch it, Own it rule. Under the NEC, if an electrician modifies an existing circuit, they are often required to bring that specific portion up to modern code. this can add AFCI protection
If the foundation is a mess of shared neutrals or bootleg grounds, an AFCI breaker will trip instantly and never reset. You can't finish the new work until the old ghosts are busted
 
Good question, and it’s smart you’re thinking “don’t stack new work on old problems.” In older homes, the first red flags are open splices outside boxes, missing grounds or bootleg grounds, mixed neutrals across circuits, and any shared neutral without a proper handle-tied 2-pole breaker. If any of that shows up, the job usually shifts to “stabilize and document what’s here” before adding new loads.
