What’s your termination routine so nothing backs out later?

I wanted to start a practical Electricians Forum thread on torque and terminations, because a lot of the annoying problems we see later come back to the same thing. Something was not tightened the way it needed to be.

How do you keep terminations consistent across a long day, especially when you are moving fast and doing repetitive work? Do you torque everything to spec every time, use a set routine, or have a personal system that keeps you from missing one?

Also, what heat signs make you pause immediately? Discoloration, smell, insulation changes, warm neutrals, anything that has become an instant red flag for you.

And for anyone who has dealt with loose connections in the field, what was the most common cause you found? Under torque, over torque, mixed conductor types, poor strip length, or something else?
 
The long day part is real, that’s when mistakes sneak in. I’ve started doing a quick final pass where I physically touch each termination point before closing up, just to force a second check. If I see insulation getting shiny, brittle, or browned, I stop and figure out why. In the field, is under torque still the most common thing you’re finding, or more like wrong strip length and poor seating.
 
I look at the conductor after stripping to ensure I did not remove strands.

Next is to torque to spec. Do other things for several minutes, then re-check The re-check is very, very important for fine strand, such as Type W.
Everything gets a tug before closing up.

If working with solid (rare for me), I stare at where the bare leaves the insulation to make sure the stripper didn't nick or groove the conductor by mistake. I also pull-push it to make sure it does not snap off.

Whenever I am in an enclosure with devices (load center, panelboard, bus duct, transformer, control center...) I check all of the terminations in the enclosure just before closing. Why? Habit I guess.
 
My routine is strip length consistent, conductor fully seated, then torque to spec and do a quick re check before closing the box. I like marking finished terminations so I don’t second guess later. Heat signs that stop me are smell and any discoloration at the lug.
