Panel upgrade mistakes homeowners keep repeating

Late December seems to bring a wave of panel upgrades and along with it a few very predictable mistakes. Some are small and some make you stop and take a deep breath before responding.

When homeowners start adding new loads near the end of the year what mistake do you see most often? Sharing these might save someone a headache before the calendar flips.
 
For many years, in what are now the "Olden Days" I worked part time as an electrical inspector for a large city, usually doing industrial, elevator, hospital and temporary power installations. On occasion I'd be asked to check a commercial or residential job.

The two violations I found most were:
A) Not using bond bushings (or any bushing) where needed.
B) Bonding neutral bars on sub-panels.

These were found under contractor as well as under home owner permits.
